Report of Death
The Report of Death is a declaration that a Filipino citizen died abroad. It can be reported to the Embassy/Consulate General by a family member, employer, company representative or undertaker.

In order for the death to be recorded in the Philippine Statistics Authority, the Report of death must be filed at the nearest Philippine Embassy or Consulate.
REQUIREMENTS

1. Accomplished Report of Death form
2. Death Certificate with English translation (for local burial/burial in Egypt)
3. Photocopy of passport of the deceased (data or information page only)
4. FEE: US$25.00
